Marionette lines, also called smile lines, form around the corners of the mouth and hang downwards. In severe cases, these folds can reach all the way down to the jawline, giving an appearance similar to a marionette puppet.
Marionette lines usually form in a person’s 30’s or 40’s. These lines form as collagen in the face naturally decreases with age. The lines can be worsened by gravity pulling the skin and facial fat downwards. The downward sag of the line often appears with sagging of the nasolabial fold and pre-jowls. These concerns can all be addressed with filler and are often treated together.
Modern dermal fillers can address smile lines in two ways. First, a skilled injector can place filler directly into the lines. This can reduce the appearance of the lines.

Marionette Line Filler: Cost, Time
The cost for marionette line filler treatments depends on the provider and the method of reducing the appearance of the lines. Filling the lines directly can take up to a syringe of filler (1ml) on each side, while a facial balancing approach may take up to 5 syringes (5ml) or more in total. Most injectors will charge by the syringe, and pricing will vary depending on the skill level and the type of filler used.
There are several different brands of filler that can be used to fill the lines, including Radiese®, Belotero®, Juvèderm®, and Restylane®. Hyaluronic acid fillers (HA fillers) such as Juvèderm® and Restylane® will fill the marionette lines, however other fillers like Radiesse® and Sculptra® can create collagen that lasts after the filler dissolves. This can help naturally reduce the appearance of marionette lines over time.
